Religious Education at
Our Lady of the Assumption
At Our Lady of the Assumption, our mission is to help families grow closer to God together. We empower parents/guardians as the primary teachers of the faith and are dedicated to walking alongside families. We provide the tools, community, and spiritual support to nurture Christ-centered homes and accompany your children on a lifelong journey of discipleship.
In partnership with the Archdiocese of Los Angeles, Our Lady of the Assumption Church is committed to your child's safety and well-being. Enrollment in our programs and involvement in the experiences we offer requires parent/guardian acknowledgement and signature. By registering and agreeing to the terms and conditions of Religious Education at OLA, you are consenting to the following, unless OPT OUT forms are completed and submitted to the Director of the selected program:
- Permission and Consent to Treat - Medical Release
- Youth Photo Release (you may OPT OUT)
- Empowering God's Children and Young People (you may OPT OUT)

Permission and Consent to Treat minors
As the legal guardian of the minor, you are giving permission for the minor in your care to participate in associated activities of Religious Education at Our Lady of the Assumption. You authorize OLA staff and volunteers to take action deemed necessary in the event of an emergency.
Photo Release for children and youth
As the legal guardian of the minor, you are authorizing the use of images, video, voice recording, and/or name of the minor for Religious Education purposes. We are committed to protecting the privacy of all involved in Religious Education at OLA. You may opt out below.
Empowering God's Children and Young People
In partnership with the Archdiocese of Los Angeles, OLA is committed to the safety and well-being of all children and young people. The Empowering God's Children and Young People program is designed to help minors know they have the power to protect themselves from harmful or threatening situations.
