Family Faith Formation


First Holy Communion and Reconciliation

Grades 1-5

Family Faith Formation is a family-centered faith formation.


Our Lady of the Assumption Parish supports parents in the promise made at their children's baptism: to raise them in the Catholic faith.

Through Family Faith Formation, the whole family grows together in faith.


First Holy Communion is a momentous occasion in the life of a Catholic child, as it brings them closer to Christ and allows them to experience the true presence of Jesus in the Eucharist. 


We wish to make our Family Faith Formation program the best possible experience for you and your children. In this two-year program, parents attend every gathering with their child(ren). We will meet in person approximately every other week with at-home activities for the weeks that we do not meet. 


The cost of the program is $200 per year. Additional siblings have a reduced fee of $100 per year (siblings = children with the same parents/legal guardians).


If your child is not yet baptized, we will assist them in receiving this Sacrament. You do not need to wait to begin Family Faith Formation.


For children enrolled in the program who demonstrate readiness: 

  • In the first year, children will receive the Sacrament of Reconciliation.
  • In the second year, children will receive their First Holy Communion.

GATHERING OPTIONS

You will get to choose the day of the week and time that your family will attend. We gather approximately every other week.

  • Wednesday 5pm-6:15pm
  • Bilingual/Bilingüe
  • (Children must be fluent in both English and Spanish)

Register for Family Faith Formation

Before beginning the registration:

  • Only a parent or legal guardian may register a child for Faith Formation programs
  • Any parent/guardian contact information provided will be added to communications via email and text via "Flocknote" regarding the schedule and other reminders.
  • Have a photo of your child's baptism certificate on your device (Year 1 only)
  • You will add this image where it asks if your child is baptized
  • Baptism records are only available to a parent or legal guardian. The Family Faith Formation office cannot request a copy from OLA on your behalf.
  • For those who are not yet baptized, answering no to the baptism question will notify us that you are seeking baptism for your child.

A baptismal certificate is

required for each child.

For those baptized at OLA, you must submit a copy as the Parish does not keep certificates on file. Only a parent or legal guardian can request a copy; the Family Faith Formation office is not able to request a certificate on your behalf. Copies may be requested by emailing the Parish Information Center at

parish@ola-vta.org

There is a $5.00 fee for each copy, and a 3-5 day processing time. Bring exact change if paying with cash. Please plan accordingly.
